History Taking

History Taking

  • The first step of examining a patient is documenting a detailed history of the complaints for which the person has come to consult the doctor.
  • This step is performed by a qualified Optometrist/Refractionist/Paramedical or doctor.
  • The medication the person is taking and in what dosage is also noted as many drugs have an effect on the structures of the eye which may cause a visual problem if undetected.
  • The details of any previous treatment to the eyes(laser or surgery) and any major systemic procedure is documented
  • The details of the glasses/contact lenses that the person uses along with history of allergy if any to medication or food stuff must be mentioned
  • In case of children, birth history details are noted.

Please Note:
  • Manysystemic conditions and the medication taken for them can affect the eye, so it is very important that you mention the details of all associated conditions.
  • It helps to carry all the old reports and prescriptions

Ocular Evaluation

Vision and Refraction

In this stage of ocular evaluation, a refraction value is taken with the help of an automated machine and then the distance and near visual acuity of each eye and then both eyes together with and without glasses(wherever applicable) is recorded. The spectacle correction is then tried, called subjective refraction to achieve the best vision with the least glass power.

Please Note:
  • Fluctuations in blood sugar value will change the refraction status, so diabetics must ensure good control before changing glasses
  • Visual acuity or vision is the amount one sees, while refraction is power of the lens in the spectacles
External Examination

EXTERNAL EXAMINATION

The pupillary reaction checked with the help of a torch light, gives a lot of information on the health of the nerve at the back of the eye.

The extra ocular movements of the eyes are synchronised movements which involve many cranial nerves and results in binocular vision and improved range of vision.

The diagnosis of squint (latent or manifest) is made through a set of tests which involve seeing the movement of the eyes.

Forward bulging of the eyes called proptosis is also detected during the external examination of the eyes.

  • The external examination gives a wealth of information in diagnosing neuro-ophthalmology conditions, squint, pseudo squint, nerve palsies, thyroid eye disease, orbital tumours and much more.

Slit Lamp EXAMINATION

Slit lamp Examination

The slit lamp helps the ophthalmologist see the front of the eye in detail with illumination and magnification. There are blue and green filters for special tests. The slit lamp is also required to measure the intraocular pressure by the Goldman Applanation Tonometry(GAT) method.With the help of special lenses the optic nerve and the macula can also be seen with or without dilatation.

Even children from the age of 3 can be examined with this equipment.

Intra Ocular Pressure Measurement

Certain structures within the eye manufacture a fluid called aqueous humour, which nourishes the structures within the anterior segment of the eye and leaves the eye through a meshwork of structures called the angle of the anterior chamber.This movement of fluid is a fine balance which results in and is required for their optimal functioning.

The normal pressure ranges from 11-20 mm of Hg.

Please Note:
  • Raised intraocular pressure(IOP) alone does not constitute Glaucoma
  • IOP may increase due to various drugs(systemic or topical)
  • Patients above 40 years, hypertensives and those with family history of Glaucoma must have the IOP measured during routine eye evaluation

The retinal evaluation is best done following pupillary dilation, which enables the examiner to see the central and the retinal periphery.
